Remote Medical Assistant – CCM, RPM & BHI Patient Support

We are seeking a reliable, compassionate, and detail-oriented Remote Medical Assistant to join our team. This role involves working closely with patients enrolled in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and Behavioral Health Integration (BHI) programs.

The ideal candidate is professional, dependable, comfortable speaking with patients over the phone, and able to document clearly and accurately.

Position Responsibilities:

Complete comprehensive monthly CCM calls with patients to review symptoms, medications, appointments, barriers to care, and overall health concerns.
Support patients enrolled in RPM by reviewing vitals, clearing alerts, following up on abnormal readings, and encouraging patients to submit readings consistently.
Assist with BHI outreach by checking in on patients’ emotional well-being, identifying concerns, and documenting patient-reported updates.
Provide patient education, encouragement, and support while maintaining a warm and professional tone.
Document all calls, interventions, alerts, and patient updates accurately in the system.
Escalate urgent symptoms, abnormal trends, or patient concerns to the appropriate clinical team when needed.
Maintain confidentiality and follow HIPAA guidelines at all times.

What CCM, RPM, and BHI Mean:

CCM — Chronic Care Management: Ongoing monthly support for patients with chronic conditions such as hypertension, diabetes, COPD, CHF, kidney disease, and other long-term health concerns.
RPM — Remote Patient Monitoring: Monitoring patients’ home health readings such as blood pressure, blood sugar, weight, pulse, or oxygen levels and following up when readings are abnormal.
BHI — Behavioral Health Integration: Supporting patients with mental health or emotional wellness needs by completing check-ins, identifying concerns, and helping connect them with appropriate care.

Requirements:

Medical Assistant experience preferred.
Strong phone communication skills.
Excellent documentation skills.
Must be reliable, punctual, and able to work independently.
Must be compassionate, patient-centered, and professional.
Experience with CCM, RPM, BHI, EMR systems, or remote healthcare is a plus.
Must be comfortable making outbound calls and speaking with patients daily.

Pay:
Pay is set at $3 USD biweekly.
