Remote Medical Assistant – Nephrology CCM Program

We are currently hiring a motivated and compassionate Remote Care Coordinator to support our nephrology Chronic Care Management program. Our team works with patients who have chronic kidney disease, hypertension, and other renal and cardiovascular conditions.

Start Date: July 27, 2026
Starting Rate: $3.25 per hour, paid biweekly, with the potential for future increases based on performance.

Position Responsibilities

The Care Coordinator will:

Communicate with patients primarily through text messaging, with occasional phone calls.
Conduct thorough monthly patient assessments.
Check on symptoms, medications, blood pressure readings, appointments, and barriers to care.
Provide excellent customer service and compassionate patient support.
Encourage hesitant or difficult-to-engage patients to participate in their care.
Identify concerns and ensure patients do not fall through the cracks.
Document patient interactions clearly and accurately.
Escalate important clinical concerns to the appropriate medical team.
Assist patients with appointment follow-up, medication adherence, and care coordination.
Learn and use multiple healthcare and communication platforms.
Ideal Candidate

Speaks and writes English clearly and professionally.

Is comfortable speaking with patients from different backgrounds.
Has strong customer service and communication skills.
Is patient and confident when working with hesitant or frustrated patients.
Is tech-savvy and able to learn new systems quickly.
Can complete detailed assessments and recognize important patient concerns.
Has reliable internet access and a quiet working environment.
Can work independently while remaining responsive to the team.
Has previous healthcare, medical assisting, nursing, care coordination, customer service, or call-center experience.

A clinical background is preferred but not required. The right candidate can be trained if they demonstrate strong communication skills, motivation, professionalism, and a genuine interest in patient care.
