We are seeking a reliable, compassionate, and detail-oriented Remote Patient Support Specialist to join our team. This role involves supporting patients by identifying health-related social needs and connecting them with helpful community resources.
The ideal candidate is professional, patient-centered, resourceful, and comfortable speaking with patients over the phone. This position is a great fit for someone who enjoys helping others, researching resources, and providing clear, supportive guidance.
Position Responsibilities:
Contact patients by phone to discuss social needs and barriers to care.
Provide resources for SDOH-related concerns, including transportation, food assistance, housing support, medication affordability, utility assistance, caregiver support, medical equipment, primary care options, specialist referrals, and insurance-related needs.
Research local, community-based, faith-based, nonprofit, and government resources based on the patient’s location and situation.
Send patients clear and helpful resource information by phone, text, or email.
Document patient outreach, needs discussed, resources provided, and follow-up actions accurately.
Help patients understand their options in a kind, professional, and easy-to-understand way.
Escalate urgent needs or complex concerns to the appropriate team member when needed.
Maintain patient confidentiality and follow HIPAA guidelines at all times.
What SDOH Means:
SDOH — Social Determinants of Health refers to the non-medical factors that affect a patient’s health and quality of life. These may include access to food, housing, transportation, employment, insurance, safe living conditions, education, financial stability, and healthcare access.
In this role, you will help patients find resources that may reduce barriers to care and improve their overall well-being.
Requirements:
Experience in patient support, care coordination, medical assisting, customer service, case management, or healthcare preferred.
Strong communication and phone skills.
Excellent documentation and organization skills.
Ability to research resources quickly and accurately.
Must be reliable, punctual, and able to work independently in a remote setting.
Must be compassionate, professional, and patient-centered.
Familiarity with SDOH, community resources, insurance, Medicaid/Medicare, or healthcare navigation is a plus.
Must be comfortable making outbound calls and assisting patients with sensitive needs.
Pay:
Pay is set at $3 USD biweekly.
